Understanding Forefoot Pain: Causes, Symptoms, and Treatments

Understanding Forefoot Pain: Causes, Symptoms, and Treatments

By Dr. Amanda Matz, DPM | Auburn, CA Podiatrist

If you’re feeling pain or burning in the ball of your foot or between your toes, you’re not alone. Forefoot pain is one of the most common reasons patients visit a podiatrist. Because this area of the foot bears so much weight with every step, even small issues can lead to significant discomfort over time.

What Is Forefoot Pain?

The forefoot includes the area just behind your toes—the metatarsal heads, joints, and soft tissues that absorb shock and help you push off when walking. Pain in this area can be sharp, burning, or aching, and it often worsens with activity or tight shoes.

Common Causes of Forefoot Pain

Several conditions can lead to forefoot pain, including:

  • Metatarsalgia: General inflammation and soreness in the ball of the foot, often from overuse or ill-fitting shoes.

  • Morton’s Neuroma: A thickening of the tissue around a nerve between the toes, causing numbness, tingling, or burning.

  • Capsulitis: Inflammation of the ligaments around the toe joints, commonly beneath the second toe.

  • Sesamoiditis: Inflammation of two small bones beneath the big toe joint, often seen in runners and dancers.

  • Bunions or Hammertoes: Structural changes that alter foot alignment and increase pressure on the forefoot.

  • Stress Fractures: Tiny cracks in the metatarsal bones caused by repetitive stress or sudden increases in activity.

Symptoms to Watch For

  • Aching or burning in the ball of the foot

  • Pain that worsens when walking or standing

  • Tingling or numbness between toes

  • Swelling or redness in the forefoot

  • Feeling like you’re “walking on a pebble”

If these symptoms persist, it’s important to have your feet evaluated by a podiatrist to identify the underlying cause.

Treatment Options

At Dr. Amanda Matz, DPM, we focus on identifying the source of your pain and developing a customized treatment plan. Depending on your diagnosis, treatment may include:

  • Supportive footwear or custom orthotics

  • Metatarsal pads or offloading devices

  • Physical therapy and stretching exercises

  • Activity modification to reduce stress on the forefoot

  • Anti-inflammatory medications or targeted injections

  • In some cases, minimally invasive surgical options for long-term relief

Preventing Forefoot Pain

  • Choose shoes with a wide toe box and good arch support.

  • Avoid high heels and thin-soled shoes for long periods.

  • Maintain a healthy weight to reduce foot pressure.

  • Stretch and strengthen your feet regularly.
